Funds are being used by Bream Neighborhood S.H.O.P. to launch a Community Re-entry and Linkage to Care Project supporting individuals recently released from incarceration. The initiative provides housing, naloxone distribution, treatment linkage, harm reduction services, and HIV/HCV testing through a dedicated re-entry specialist. By addressing housing and treatment gaps for one of the state’s most at-risk populations, the project works to reduce recidivism, homelessness, and fatal overdoses while strengthening community health and safety.
