Award Summary:
Funds are being used to implement a targeted housing and reentry initiative for justice-involved youth and young adults at high risk of homelessness across Region 4. This investment supports coordinated discharge planning by deploying mobile Coordinated Entry specialists inside correctional and juvenile facilities to assess housing needs, connect participants to stable housing, and coordinate referrals to recovery and support services before release. Resources integrate data-driven strategies with direct service delivery, strengthening partnerships with juvenile justice and reentry programs to address critical gaps in the transition process. By combining housing stability with recovery-focused supports, the initiative reduces homelessness, lowers recidivism, and improves long-term outcomes for youth and young adults leaving justice involvement.
