Award Summary:
Funds are being used to expand community-based reentry services for justice-involved individuals with substance use disorder through Southern Highlands’ coordinated QRT, LEAD, and Angel Initiative. This investment strengthens immediate transition support from jail to the community by adding dedicated case management and peer recovery staff to develop individualized reentry plans, coordinate services, and provide ongoing mentorship. Resources also support implementation of the Situation Table model to enhance community crisis response, creation of a safe de-escalation space using the Living Room model, and distribution of barrier-reduction funds to address urgent needs. By combining wraparound supports, coordinated crisis response, and targeted reentry resources, the initiative improves continuity of care, reduces recidivism, and promotes long-term recovery and stability.
