Award Summary:
Funds are being used to expand the REACHback Reentry Navigator Program, strengthening recovery-focused reentry supports across additional counties in Region 5. This investment integrates trained Navigators with lived experience of recovery and incarceration into a coordinated recovery ecosystem, providing hands-on support before and after release. Resources fund pre-release planning, assistance securing identification and benefits, connections to primary and behavioral health care, job readiness training, and intensive housing support. By bridging critical service gaps during the first 30 days post-release and working closely with parole, healthcare, and housing partners, the initiative reduces relapse and recidivism, increases stability, and expands a proven, data-driven reentry model across the region.
